There's at least one bank don't need name or any ID to obtain prepaid cards - on card is only embossed "KARTA PREPAID" which stands for "PREPAID CARD".
You get leaflet with IBAN acct no., sealed PIN, card itself glued to leaflet.
These cards have payWave functionality so you can pay with them contactless.

Each pack contains: card, IBAN account no, PIN, information flyer.
Managment of card is available through online-banking SSL site.
Currently I have batch of:

Daily payout limit from ATM - ~150 EUR
Daily payment limit - ~150 EUR
Yearly payment limit - ~2500 EUR
Yearly topup limit - ~2500 EUR
Total balance limit - ~2500 EUR
ATM withdrawal fee - 3%
