I'm using an image from slushpool.  I made a rookie mistake. The SD card had been used before. so what I did was I reformatted it. And reflash the image onto it. That seemed to work. Thank you for everybody's help. I wish you all much success in your mining efforts.

I'm having the red and green LED constantly on when I put in an SD card (yes I changed the jumper). When I take the SD card out it boots up fine with factory firmware. The controller board is v1.0.

Are you using the SD card image from Bitmain which is for s9?

Try to use the image for t9 under "t9+ -SD Tools.zip" or if it doesn't work try to use the s9 recovery from this link below.

This happens mainly to one of these reasons

1- The Sdcard you use doesn't work:

For some weird reasons, you might need to try a couple of Sdcards to get one that works with these control boards, once you find one, label it and hold on tight to it, also try to use smaller size Sdcards as they tend to have more success.

2- The Sdcard slot is dirty:

Just blow at it with your mouth, that should clean any dust in the way

3- The component that is responsible for the flash is bad:

This is more like physical damage that you can't fix.

Also, it's possible that you did not load the files the way you should, so it would be best to test on another control board if you have one.

I think much better to do a hard reset first before you do anything.

Here is the guide how to hard reset your miner

  • First restart your miner then wait for 2 to 3 minutes
  • After powering on the miner for 3 minutes, press the reset button in your miner for 5 seconds then release
  • The miner will restart it's own automatically for 5 minutes
  • Then try again if the IP reporter detects the miner

Sometimes resetting could solve this issue, but if this guide won't work possible you have a bad firmware or hardware.
Could you try to replace the cable from controller board and check if your issue solve.
 
If your miner still under warranty better to contact the support and send it back to Bitmain.

Ive seen this happen before, no rust issues caused it though. In my experience, most of the controllers that end up this way first present issues by factory resetting themselves for no good reason. I am assuming its bad flash memory on the controller. I just throw em away and put a new controller on em.

As soon as power is applied to this Antminer S9, both of the red and green status LEDs turn on solid (not blinking). This is similar to the effect produced when the IP Report button is pushed in, so I checked to make sure the button is not stuck in. The miner does not request a DHCP lease so there is no way to log in. There is only one LED lit on the controller board, and none on the hash boards. This model has a microSD reader but I doubt that would help.

How could this problem be addressed? The only thing I could think is that since there are some signs of rusting on the fan grate, the IP Report button circuit might have rusted closed.
