So far I see two debit cards for Americans, bitpay and shift (coinbase.) Are there any that require no verification?

Is it possible to use a virtual one and make purchases still in the US?

On https://uquid.com/ there is virtual feature but i don't know it is worth at USA or not because of i am not from USA.

It will be hard for you to find some card that doesn't require identification and some personal data. Conditions is all services are harsher then before, there is certain pressure on them because of money laundering and terrorist threat. So if they want to run legitimate business they have to obey certain rules.I beleive this especialy like that in US.

1. There are debit card services with minimum verification like email and mobile number. Almost all providers support USA, check here and find one : https://bitcointalksearch.org/topic/list-of-bitcoin-debit-cards-1242788

2. Virtual cards are limited to online usage. Except this there will not be any other differences from plastic cards.

3. Usually preloaded cards are made for one time use. There must be some exceptions too.

A virtual card is no different from a plastic one since the same bank issue both cards most of the time. Only service that I'm aware of and offers a plastic card for U.S residents without any kind of verification is CoinsBank but the card have no name on it and there is only a possibility to put a maximum of 300$ only.
