I don't think there's an exchange with no issues; all exchanges experienced some issues unless you're talking about non-KYC exchanges, you can only find them on the link provided by dkbit98.
However, if you're talking about centralized, I discourage you from using OKX if you can't afford to lose your BTC by depositing below 0.0005 BTC; that's my first issue that I experienced on using their exchange.
You can check these 4 exchange that I use: Binance, Bitget, bybit and Coinex
I haven't experienced any issues yet with using them, but expect the KYC to be mandatory. If there's no problem providing them, then that's a good exchange that I can recommend.

You have to be a bit more specific.What is best for me maybe is not best for you or for someone else.
Safest option is to use decentralized exchange like Bisq, and if you want quick swap than I suggest using eXch.

First of all, as examplens already mentioned, you need to specify what kind of exchange you're looking for, is privacy your main concern, or convenience? For centralised options, Binance and Kraken are your safest bets, other alternatives include Bybit or crypto.com. I would advise you to set your needs and priorities before making up your mind. Read around the forum as well, this question has been asked numerous times, you'll find plenty of information and answers without waiting for new responses.

There is no universal answer to these questions and much depends on your needs. Are you looking for an exchange for trading or just for currency swap, is it important for you to be no-kyc, CEX or DEX, which region are you from and are there any restrictions...eXch.cx imposed itself as one of the better solutions, taking many parameters, although this does not mean it will be ideal for your needs.
