Go to fiver.com, you'll get lots of card designer over there. you can pay via bitcoin also on fiver. no matter if designers are not related to bitcoin. just ask them what you want and they'll give you best thing you need..

Send me what details you need, and I will make one for you. If you like it, you can pay me. If you don't, you don't have to. It's a hobby of mine, but it relies on your honesty
