I think BFL did this. I am not sure if they went through any kind of verification process though

It seems that your from Indonesia and your best bet would be third party website for this matter although you need to be really careful as scammers are all over the internet ... Here you could find a local seller that might help you: https://localbitcoins.com/      You could also try an online local exchanger if it has, I couldn't find any from Indonesia though

Usually any transaction to banks is needed at least one or two kinds of verification. I do the same in my country and the verification process is a must.

To transfer bitcoins to a bank without verification, you would need to exchange with a user from the forum else you can use an exchanger on localbitcoins or ecoin.eu but it needs you to verify the account but it's a safe method. LBC is risky and unless the user doesn't have a good trust rating, you shouldn't risk your bitcoins.

This depends on the amount you want to transfer because you must complete your verification process if you want to transfer above $5000, notwithstanding you can achieve this transfer using most bitcoin exchange that transacts USD.

I think you can only transfer to a bank account registered with your name. You can't send it to others account. I didn't see this option in any exchange or bitcoin payment websites. Bitpay is for receiving bitcoins from payments on your store to your bank account.
