I got response from their support and they confirmed that, yes, there will be 2 conversions = 2 fees in this simple case

I suppose they will remove their exchange fees from your btc balance and then convert the rest to dollars for you. So the USD you'll see will be 0.45% less than its btc equivalent.

What I would like to know, because it is not entirely clear to me: If I deposit Bitcoin directly to a $-card (for example) and than withdraw Bitcoin from it again I will be paying fees 2 times?
no you dont have to pay the fees two times. you will just have to pay the fees while exchanging but the withdrawals are made free as they already said in that blog post. btc exchange rates will be 0.45% from 1st august while withdrawal will be made free from 1st august. i explained you from what i read in that blog post but please note that i have not used that exchange site personally.

It's a little confusing, because when I deposit BTC on my USD-card I will be holding USD. so is this an exchange for them or not? that's my doubt
