You might want to check out http://cavirtex.com/

They are a canadian exchange.

+1  For Canadians this is the best way to go.   It has cost me nothing to move CAD onto the exchange from my bank( credit union )
and nothing to withdraw CAD back again.  They just recently added a flat rate of $3 for deposits but that isn't much compared to
what it costs to get money onto other exchanges.

Its to bad that www.spendbitcoins.com doesnt work in the US anymore.

If you buy selling coins mean withdrawing $$'s (or any other currency) to your ordinary real life wallet
there are a few possibilities.

- Most any exchange allows you to move money to your bank account (government can see)
- Use a "giftcard/creditcard" site (not very traceable)
   - www.btcbuy.com
   - www.fastcash4bitcoins.com
   - www.bitinstant.com)
- Buy stuff and have them delivered from any btc shop.
   - www.btczone.com
   - coinabul.com
   - Many more here


These are just a few options but they might give you some pointers.

Obviously not eBay...is it best to go through Mt. Gox or similar? Do the exchanges buy directly or do I transact with the buyer after making contact through the exchange? 

The factors that matter to answer your question include:

 - Where are you located (country)?
 - How much are you looking to trade?
 - What type of cash are you looking to receive?
 - How soon do you need access to the proceeds?
 - Is privacy important?

I used to use bitmarket.eu to sell coins. It's an alternative to mtgox that I would recommend. I was selling to people for paypal though, something I would not recommend doing these days.

Canada.

EBay has worked well for me in the past when selling digital goods, because it puts me in touch with buyers who I more than not convince to give me a gift payment instead. Works well.
