It means you're using Trustedcoin 2FA and you need to restore it as a "wallet with 2-factor-authentication" and also type the 2FA code to it from whichever Authenticator app you're using.
That's not how restoring a 2FA wallet works. If you have the seed phrase, then when you restore the wallet you are given the option of keeping or disabling the 2FA. At no point during the restore process are you asked for a 2FA code. If also doesn't matter if you select "Standard wallet" or "Wallet with two-factor authentication" when you are restoring - when you enter your seed phrase from a 2FA wallet, Electrum will detect that it is a 2FA seed and proceed as such regardless of which option you have chosen.

Note that if your mining rewards address is not in the list of addresses shown in Electrum, and the seed phrase (in the menu Wallet > Seed) is the same as the seed you've already written down then restoring again with just the phrase won't help. It has been several months since I used Electrum and now I have what I explained in post of receives and change in a list that says its connected to server. No history and 0 balance.

I don't see where any of the addresses correspond with my pool payout, but I'm not sure how that works just yet.
If you are opening a wallet which shows no balance, no history, and different addresses to your previous wallet, then you are opening a different wallet. This is not the same wallet that you were using on your Mac.

Will it hurt to restore my wallet again? Should I move the files from the current installation location of Electrum if and when I restore?
No and no.

If you still have the seed phrase from your original wallet, then restoring from this seed phrase is going to be the most straightforward way of recovering your wallet. If you restore from this seed phrase and the wallet is still incorrect, then you have either used the wrong seed phrase or there is an additional passphrase you will also need to enter.

Yes, I did receive funds in the old version of wallet. I could not update Electrum within my OSx and Electrum would get stuck when receiving. I did have a history. After restoring ( i was going to move the file but had issues and just did a restore) to win8, Electrum loaded fine with my seed info and I did a single send transaction that worked. It has been several months since I used Electrum and now I have what I explained in post of receives and change in a list that says its connected to server. No history and 0 balance.

Looks like a completely different wallet file, Electrum wont suddenly delete all of the history from the wallet.
Having a different set of addresses based from the "missing payout address" supports this.

Check the wallet directory (loc) using the menu "Open" in Electrum if there are more than one wallet files there.
You might also want to consider renaming the wallet file or new wallets into a specific name like "miningpayout_electrum1" to minimize confusion, it'll work just fine.

Did you select the correct wallet file to open? So you're saying that you've restored the wallet using the seed months ago, left it there and you can't see anything on it now?

Nope, it's fine. Just go to File and New/Restore and select the options to restore your seed.

First, you haven't told us if you ever received funds in the wallet before moving it or not. If it had ever received the funds, you should have been able to see the transaction history after importing your wallet successfully otherwise it will be blank

I don't know where to start to figure this out or should I just try to create another wallet?

The addresses you see marked as receiving for addresses that will be generated whenever you want to deposit the Bitcoin. The change addresses are those where your change will automatically be sent to in case all the BTC is not spent in a transaction

You can restore your wallet using the wallet file or its seed. You don't have/can't import both of them.

I recommend restoring your wallet from seed since it's easier.
Download and install the latest version of Electrum on your Windows device (download it from the official website and verify its signature) then follow the steps described in this guide:

Hi folks,

I received some good advice in how to move my electrum wallet (3.x.x to 4.0.9)  from an older mac to my windows 8 setup.

I thought I completed this action successfully but I show no history. In addresses tab I show many addresses receiving and some that show change. When moving from mac to windows, I copied the wallet file, moved it to windows, setup the 'same wallet' from my seed. Which seemed to work.

Please, any help will be much appreciated.

I just got a Ledger and I would be glad to put all btc on that but not sure how until I get Electrum to work.
