Next time use Authy instead of Google Authenticator, it saves your 2FA data on the cloud, so no problems if you lose your phone.

It's impossible, i guess you need at least the address that was used as a sender or the destination address. I never saw someone who has putting amount or the time and he can find the tx address. it's impossible for me personally. Sorry, I'm on mobile.

You have the time of the transaction or time of the confirmation?
You can just go looking in the blocks timestamp and find the probable blocks. You can do it in etherscan. It will take some time, and you need to do it with exact amount. You can use Ctrl F (find) to search for the exact amount.
You gotta have a lot of time waste of searching if you just had the amount of eth on the transaction and the exact time where the transaction submitted on the blockchain. From where you sent eth?

Just insert your transaction in the Search box and it will show you the amount, fees, time, etc...
for example
