I've exported with different print-to-pdf drivers, but second is strangely rotated (it should have all the data though).

I have my pc at repair service and need to check on smartphone that PDF form from official U.S. Patent Office website. Here the link: https://www.uspto.gov/sites/default/files/documents/aia0014.pdf
If you are afraid of any viruses - check official USPTO website manually via google and then compare with my link.
But my mobile PDF application does not support such complicated interactive PDF forms. Can somebody download and export that PDF as empty print ready PDF?
