I completely fixed this by running two instances of the miner.  One amd and one nvidia.

BSOD

I found your problem. BSOD means you are running Windows. A common mistake among novice miners. Switch to linux and all your problems will go away. I have multiple 12 card rigs so I am speaking from experience.

Maybe that's what I'll tinker with next time I have some spare time.  For now I'm satisfied with a stable 179MHS/750W for each rig.

I was pleasantly surprised to notice that the 12-card rig popped back online.  I ran the miner for a bit with 8 cards active, 6amd+2nvidia, and it was smooth and stable.  Now I'm running with 10 cards active, 7amd+3nvidia.

So far going smoothly.  I'll try 11 cards next.  8amd+3nv.

liife threw a tempest at you? be a coconut !
so it begs the question, if you have 2 working units why combine?  Wattage savings is minimal.  Go back to 2.

I've been running two 4amd+2nvidia rigs for several months now.  One was on a TB85 that I already had set up, the other was a later add-on with a TB250 mobo.  I finally put all of them on the TB250 pro board.

With all 12 cards active, it is unstable, freezes, BSOD, etc.  So far I have confirmed that running in either -platform 1 (amd only) or -platform 2 (nvidia only) mode is stable.  After that I backed off the clocks a bit more and gave it another go with all 12 cards active.  After that the rig went offline and I won't be able to visit it until later tonight to reset.

This is driving me pretty crazy.  The fact that all this hardware has been running fine for months rules out a lot of the common/simple issues that arise.  I donno if maybe I got a bad TB250 pro board that can't really handle 12 cards like it's supposed to.  Unless someone has some good ideas, I'll probably just have to go back to two 6-card rigs.
