It's all about the game, and how you play it
The closest to a standard way would be placing it in a slot not occupied by anything else (ex a micro atx motherboard in a full tower case leaves room beyond the motherboard where there are expansion card punchouts) beyond that you are unforutnatly "on your own" as to how you're comfortable mouting the cards although there are some caseless guides here if you're considering a dedicated mining rig.

If using an extender cable, I suppose I need to secure the card in a different part of the box - is there a standard way of achieving this, or should I just make it up as I go?

Yes, two more cards are possible given the use of one each PCIe x4 --> x16 and x1 --> x16 cable extenders and a suitable power supply. I use Cablesaurus.com for my source on PCIe cable extenders.

I have an Asus M4A87TD motherboard with a 5700 gfx card. I'd like to add another graphics card for mining.

I understand my motherboard has one PCIe 2.0 x16 slot, (currently the 5770 will be in there), one PCIe 2.0 x4 slot, and one PCIe 2.0 x1 slot. Does this mean I could add 2 more cards if I wanted to?
