Confirmation is exactly needed to avoid trusting the other part of providing their part of the deal..
You can speed up transaction by increasing the fee you give to miners, and you can (in some cases) use exchange vouchers like the one on btc-e and transfer bitcoins
instantly w/o the need for confirmations because it's within the centralized body (exchange itself)
