You can get a Paxum debit card, then withdraw from TH or MtGox to Paxum. I know, it's not quite the same, but it works.

A Paxum debit card is pretty expensive imho.

For smaller accounts, you're probably quite right.

This was the main reason many signed up for paypal, to order items online because they either had no credit card or sought an alternative. Is this (legally) possible? If so what is holding them back from implementing such a service? It would wipe out the need for spendbitcoins.com and alike businesses.

You could either spend from BTC balance or USD balance. If Mtgox already offers usd vouchers I don't see much difference in a virtual debit card.

This would also alleviate some of the demand for BTC -> paypal trades as it leaves fewer reasons to use a paypal account if you've got a gox/th.
