Try Google forms, They're as easy as 1-2-3 and you can add form to any website and you will get replies send to you in a compiled spreadsheet.
are you needed to the admin area for showing any data on admin area or just email and database?

I'm currently looking for someone who can do a little work for me.
I think it can be done in less than 2 hours.
It's a simple web page with a form, the user compiles the form and when he clicks "Send" the data in the form are sent to me via mail or saved in a database. Like name;surname;number;interest;dateofbirth
