Thank you for all of the detailed feedback. I heard back from my request to MyRig so I currently have a ticket open with them and am planning on shipping out the power supply next week. I will keep you updated on my experience with them so the rest of the group can use it as a reference.
I also found that this model of power supply can be bought from Bitmain for $134 + a monster shipping charge. They are currently out of stock, but I also saw that they were requesting a questionable amount of shipping information for the order. I am digging into this a little more next week so I will keep you updated on that also.

Not sure about your current location but you might want to try "MyRig".- According to them, "it costs $15 to repair a power supply from Bitmain".
- They do have an "inactive" representative in here [MyRig].
