milliBitcoin when spoken.  When typed I usually used mBTC.

I am liking the "millicoins" in thread though.

millicoins seems least painful so far...

I'd use the "mBTC" notation, and verbally say "milli bitcoin" or even "millicoin".

What is the acceptable and pronounceable way to say "mbtc" in a conversation? Is there a general consensus?

"M Bitcoin"
"Milli Bitcoin"
"M B T C"
"One Thousandth of a Bitcoin"
