Unfortunately the error message appears to be an extremely vague and useless one, so it's hard to know what exactly is going on just from that...
It seems that others have had trouble with 4 GPU's. This poster got it to work by putting the first two in a Crossfire configuration and dummy-plugging the other two: http://forum.bitcoin.org/index.php?topic=7542.msg110802#msg110802

I am trying to get my mining rig up and running, but am having some serious trouble with the 4th card:
Relevant stats
Windows 7 64 bit sp1
Catalyst 11.5b
Python 2.6
Numpy 2.6
pyocl 2.6
pycuda 2.6
I have 4 gpu cards on my motherboard xfx 6950's 1GB ram models to be exact.
They are in cross fire configuration and 3 of them have dummy plugs and are plugged into pcie x1 slots via cables.
Windows can see 4 adapters in device manager.
When under "screen resolution" I choose to "extend the desktop" to the 4th card it becomes visible to catalyst.
Presently my crossfire options have disappeared from the peformance menu in catalyst, this is very odd.
So when I extend the desktop so catalyst and I assume open cl can see the card and I then attempt to run the 4th miner client in the phoenix miner gui or the "windows guiminer"
I get the following error:
Also any idea about why the miner crashes when I enable my 4th card?
