You should let the buyers upload their public address QR so it can be printed on the T-shirt below the bitcoin logo 
The "setting" fee would have to be charged individually to each shirt, so a company like teespring would charge ridiculous price per shirt

I like those t-shirts but I think if Satoshi Nakamoto will design a bitcoin t-shirt it will be plain black. As he like being anonymous :-)
