All you need is wallet.dat. Just replace your wallet with the old one. Just move your current wallet somewhere else or just rename it something in the same folder and it'll work fine.
Yes, you need to install bitcoin-qt client and copy this wallet.dat file into bitcoin folder.
In August of 2012 I bought & spent a few BTC. The next month I forgot about my bitcoins when I reformatted my computer.
