USD and the service is fully managed by a 3rd party i.e Enterprise solution,ops,finance,fraud,legal,marketing etc. however this does not include the Cryptocurrency part
The cost should go down depending on the teams you have at your disposal to manage the different aspects of the business so if you only partner up with a Issuer for the card and manage the rest of the operations in house then the cost would be around 2 Mil or less

There is lot of connections required, the project must be flawless, you cant get companies like visa or MC on board without having a perfectly structured project and team in place.
