After reading many sites including this one on GPU mining gremlins, one additional thing to try especially when changing the GPU card configuration:

1) Go to device manager.

2) Select and view display adapter list.

3) Go to view and select "Show hidden devices"

I delete any additional driver showing as hidden.

Doing this makes reinstalling drivers easier and I have found that driver crashes are reduced. I believe this especially important in mixed card configurations.

This has always been a windows issue. Those of you who use USB or RS232 devices know that windows sets up a new driver anytime a RS232 or USB device is installed in a new port.
