And I can get some sort of refund if this is a system error, which I whole heartedly hope it is.

As written by others I also think that this is a unit mixup.
And you can check it very easily by checking if the remaining fund in your wallet is USD 120 (3x40) lower then what you expected it to be.

As for refund request.
Once transactions are in the blockchain they can't be changed or refunded.
If you sent someone too many bitcoins you can kindly ask him to refund you but you can't force him.

In case of fees it would be even more difficult because the fee is given to the miners that secure the network and you do not know who they are.

Nonsense.

$4 = 0.0093 BTC (that's 9.3 mBTC)

Not 0.0000814 BTC (0.0814 mBTC)

0.0000814 BTC is only $0.04

Possibly, but probably not.  Perhaps if you gave us something useful to investigate with we could help you figure it out.  For example a transactionID or the address you were sending to and time that you were sending the transaction?

As already explained, you probably have your units mixed up.

Check your units. Electrum by default has its currency units set as mBTC. This means that it is actually 0.0001131 BTC
