What a stupid waste of money. The UK doesn't have a working contact tracing system, and it is being reported that 80% of tests are inaccurate, and any action will be too late anyway. The tests don't check for immunity, and most people should be immune by now, unless they have been self-isolating of course. There is a £1,000 fine for restaurants that serve people who haven't supplied a form, but they aren't required to check the details.
The forms are pretty useless anyway. They ask for a name,and the date and time of entry, and also a telephone number and a count of the number of people in your party ( but they don't have to fill in forms). If they think that you have been near to a person with a positive test, then they will notify you, and you have to self-isolate for 14 days. It's madness, as you could have been sirring 50 yards away from the tested person, and there is no way they can check for proximity. I reckon it is just a way to collect phone numbers to spam with their fake virus warnings.
I wonder how many people will submit details such as Boris Johnson, 10 Downing Street, and will he have to self-isolate if the alert is triggered.
firstly the amount of people that had the virus is still low. (try learning math)
secondly if a restaurant is informed a patron tested positive for covid and thus other patrons were at risk. then it is the restaurant that then hand over the patrons that were in the establishment the same time.
(not all patron for all days)i know you want to think that all details are handed over to NHS the instant you write it down. but that is not the case
also the one contact number is because the hope is that one contact will then themselves contact the other people they came with.
thus it defeats you 'government surveillance' cries because they are not gathering numbers of all your party nor gathering it when its not needed. nor gathering it for non covid tracing uses
in short if a restaurant didnt have a covid infectee in their establishment the restaurant doesnt hand the number to anyone and is binned in a few weeks

restaurants wont ask for home addresses. they only ask for a name and telephone number. so all you need to do is put a realname[or] nickname and a phone number.

its not 80% inaccurate(false positives) its 80% accurate with 20% saying negative when it should be positive
this 20% discrepency is not a test fault. its actually only because some people are asking for test before getting symptoms so the amount of virus in their system is too low to measure, or they have bad hygiene and it has contaminated the sample
having symptoms and testing to see what it is, is a accurate test. getting tested with no symptoms is stupid because the virus may not manifest into anything to worry about in some cases. and may manifest into something but your just too soon to tell. thus wasting a test
this is why in the UK they are trying their best to only test people with symptoms to get the most accurate results which are above 90%

not even 320k if you were to exagerate a 1000% 'asymptomatic' number
the peak had a 6k. so using your exagerated asymptomatic of 1000% thats 60k
if the peak stayed for 200 days that would 12mill =20%
20% infected at a maximum exageration of constant peak that lasts 200 days
reality is below 10% if you tried to do some maths. even with the exageration 1000% but with using actually daily counts
