Been using an accounting firm based in New York for a few years now, and they do Bitinstant's books as well.

Also, Ron Lazar of AINI & LAZAR PLLC is our legal counsel and board member, I've spent the last few months educating him on Bitcoin and at the same time paid him alot of money for research into financial laws.

He helped us put together our 16 page AML manual as well.

I remember at one time Mt. Gox describing a report it could run to provide those numbers, ... on a FIFO basis, I believe.

This gets difficult when you use multiple exchanges.   I can't imaging trying to manually do the tax reporting for someone doing arbitration between the exchanges, for instance.
