The best and most reputable (in my opinion):
Bitfinex.com
Kraken.com
Bittrex.com

Though USD withdrawals on Bitfinex and Bittrex would be in Tether(USDT). You can look at the exchange rankings on Coinmarketcap if you want to check out the long list of exchanges, just use your browser's find feature(CTRL+F) to filter in the BTC/USD pairs. Personally I'd stay away from Poloniex though since I've heard their customer support is pretty shit. Goodluck.
Yes you mentioned very much reliable sources which can help you out in converting Bitcoin / USD Cash. In addition to them, you can also try Gdax and Bitstamp as well.  I have heard that bitstamp is most reliable. By the way with the total volume of trade I like it the most.

There are very few delayed withdrawals of BTC if you request it from the site wallet, unlike many of the exchanges listed on coinmarketcap. And online cash payments are only as slow as the method (as I mentioned, instant in the case of my preferred choice of payment in my country). It's P2P, btw.

What is this "delayed withdrawals" Huh  Why does that happen?

This better not jinx LBC! None of my withdrawals from localbitcoins were ever delayed longer than a couple of minutes. Most happen within 60 seconds of my request (transaction broadcast anyway).

With other exchanges I've used in the top 10 of that list, it has taken anywhere from 5 minutes to 3 days if I recall correctly. Judging from complaints online, you'll find many others with the same gripes. I'm pretty sure there are a lot of factors. Verification levels and withdrawal amounts being two. But since none are genuinely transparent, one can only guess at the reasons.

DaMut's given you the biggest exchanges in terms of volume, and this is also my preferred way to choose an exchange. But it's a bit different for Bitcoin, I would say, in terms of reputation. Localbitcoins is the one I use most frequently, partially for its ease and close-to-market-average prices, although lately its' been dropping off ever since BTC broke the $4,000 barrier. But mostly because of its escrow-system, instant trade resolution in certain cases, and reputation. Can anyone advise me as to the three  biggest exchanges?  Only for Bitcoin / US Dollar (BTC/USD) exchange.
At this stage, I am not interested in  any of the Alt currencies though I may be interested in Ethereum later.

Top 3 'the biggest exchanger' based on vol for BTC/USD and Vice Versa in my view for now:
Bitfinex,
Gdax,
Bitstamp,
you can go here for more information about it and control it everyday because it can change at any time
https://coinmarketcap.com/currencies/bitcoin/#markets

i don't list Poloniex because it's not really safe to use and many people have a problem in there,
so try to avoid this kind of exchanger for now because trading is a high risk activity and doing it in a shady exchanger make it even worst.

but the better exchanger in my opinion for doing a daily trade is Kraken,
because i never heard they have an attack from hacker just like Bitfinex and Bitstamp
