but we never found out because it's being abused in small amounts?

Good thing Bitcoin wasn't popular at the time this happened.
Or good thing that it wasn't Ghash.io that exploited the bug! It would be hard to correct now if they didn't want it corrected!
Of course they don't want to destroy bitcoin like something like that would, but if a similar bug was found they could exploit the small amounts option.

In short, that block 74638 was orphaned before getting 120 confirmations (Block rewards can't be spend before getting 120 confirmations).
