Bitcoin Difficulty Estimator gives you estimation for a next difficulty change. It's estimation with wide range of lower and upper change estimations. Estimations for next change is from 4% to 13%.

In short: difficulty is stored (in compressed form) in header field "bits" and difficulty adjusts every 2016 blocks.

More precisely, the target is stored in the nBits field in the header. The difficulty is a value that is computed from the target.
