There were also the 2 series (3 notes total) of the XMR (Spesmonerojo) notes made in collaboration by phishead and Polymerbit

And then also the Emergency Currency / Delayed Delivery notes... all though, those were not technically "loaded", but more of a gift card to be redeemed for product in their on-line store

Yup. there were the monero notes and the others you mentioned as well.

There are a few private commission Polymerbits which were funded:

Etoro

20
21
50
100
250 pound sterling in BTC

20,000 pounds total funded in 2018.
---------------

Private notes under partial NDA up to 10 BTC loaded.

Who knows if they will be in the market in the future.

Edit: There were also the certificates

1. Penguin Note
2. Red Cardinal

.... And the collab between coldscam and polymerbit for the Ten year BTC anniversary notes. (Which are still intact, yogg never kept those keys imo).

Not sure if there were more, but some were sold with keys so you could fund them. I probably have a couple I funded myself at some point.
