Try Lykke, 0% fees, and you can trade from the app.

I'm also using Bitstamp and Kraken (+localbitcoins) to cash out and Bittrex and Liqui.io for trading and altcoins.

Alternatively, although more expensive are the "national" exchanges, which work very well to buy and withdrawal directly into my bank account but I'm only using them when I'm in a rush.

Already a lot of good answers here, but yes the basics is getting bitcoin or ethereum from Coinbase and then transferring to your desired exchange.  I'm using Bittrex and Poloniex right now and although I like Poloniex's UI and trading charts, Bittrex is much more stable (and I have more confidence in their future.)
so if OP is wondering about the fee maybe its better to use the alternative, you can buy eth from coinbase then send it to bittrex or poloniex if you are planning to trade some alt coins, maybe you can afford to save some fees using eth than btc then convert to btc to buy alts but I'm not sure if that can be useful just my
two cents though.

if you have a coinbase account go to gdax.com, there you have 0% maker fee and you can move everything from coinbase to gdax for free (and back to coinbase...i use it to cash out via SEPA to €)...Its the same company. Sadly still only ETH, BTC, LTC but you might save some fees.

Poloniex is good for daily tradings. If your trading volume is very high, the fee is decreasing. Bitfinex and kraken are good for usd cash out.

I also bitcoin as my base coin. My all time favourite exchange is bittrex in that matter. I trade from btc to other coins in general. I sometimes go buy tether to cut losses but I dislike to go cash. Bittrex is the best exchange. Oh also nova was quite good.

I always use BTC as my base, i.e. buy BTC & then convert the proper amount to the selected ALT (using mainly shapeshift) including Eth (& then further to these ERC-20 tokens)

To cash out my BTC & even buy some; it's always Bitstamp for me...

what strategies everyone is using to purchase their altcoins?

I started with coinbase to buy bitcoin and transfer them to bittrex to buy most of the other altcoins but since Coinbase' fee is 1.5% which is very expensive. If you buy and sell 2 bitcoins for $4000 each, you end up paying $240 in fees.

Since I have started to buy more now, I want to know what others are doing. Also which exchange do you use to cash out your bitcoins?
