The easiest way without connect or disconnect your GPUs is using the command line -di ( -di 0123456 means all GPUs are disabled, -di 012, means GPU0, GPU1 and GPU2 are disabled) and watching physically wich GPU fan is working. Hope It helps.
What I do is during setup, I only add 1 card at a time. I use GPU-Z and find the ASIC quality, and I print a label and I put that on the front of the GPU. That way I know exactly which GPU corresponds where via the ASIC quality.
Then, while mining, I use the numbers, 1-6, to turn the cards on/off and look at the card that goes to 0% utilization. Then, in HWInfo, I relabel the cards with their Claymore order and ASIC%.
It's a PITA, but it works.
The newest version of Claymore (9.6) now automatically follows the same GPU order as HWinfo/Afterburner.

The only way to be sure is to look at the GPU's and see which one has the fans stopped. The GPU index from Afterburner and Claymore often don't match.

I find the Claymore miner GPU indexes to be inconsistent at best. The easiest way I have found out to determine which GPU index reported by the miner corresponds to which GPU is to set the fan slider all the way down to the left in Afterburner. That will stop the fans on the card and you can see which GPU and GPU index shows the 0% fan speed in the miner. You have to do it quickly because the card will heat up fast without a fan going. The other way is to set the fan to 100% and do the same.
I'm having problems with my Claymore ZEC miner. One GPU is clearly screwing things up, but it's a nightmare trying to determine which one. Is there a way to know which GPU is which? As in, how can I derive which PCI slotted GPU is responsible?
