fastcash4bitcoins.com is extremely reliable and quick, although you will get slightly more for your coins if you sell on an exchange like MTGOX.

It depends on where you are located.  In many areas you can find some really good deals with LocalBitcoins.com

If you can't find anyone local offering a good deal, you can try creating an ad at LocalBitcoins.com that offers a better deal than anyone else in the area.  Someone else who is looking for a fair deal might find you.

Assuming neither of those deals work out for you, fastcash4bitcoins.com and MtGox are both options.

MtGox requires identity verification (which can take several days if you haven't already done it), and it can be a hassle getting your cash from them after the sale.  You'll also pay a transaction fee on the exchange, and you may have fees associated with getting your cash transferred to you.

fastcash4bitcoins.com is very convenient.  You'll get your cash fast, and you won't have a lot of complications.  You'll pay for that convenience with an exchange rate that isn't quite as good as the rate at MtGox.

You could try btc-e while waiting for Mt. Gox they do BTC/USD trades.

I heard Intersango is good but never tried them myself.
