Another issue are regulations. Over here, you are practically a bank, if you accept fiat. The regulations then are x times more strict and complicated.

So many exist because all are crap, if a good one came along all existing would die.

Yes, but what I am wondering is why one hasn't emerged as clearly dominant overall.

Cryptsy is the most dominant, I think.  But it is regional.  Some people prefer more localized ones.  There are too many coins and the exchanges try to fill a role as being the defacto for the coins that they host.  Exchanges are cheap to run and easy to spin up.  Cryptos are only about 3 years old.  The flood of altcoins are only about 10 months old so expecting a dominant exchange after only 10 months of existence is a lot of ask for.  Lets see what happens in 3 to 5 years.

most of them are small exchanges that add newer coins - with all the new coins that have been started over the past few months I'm surprised there aren't more exchanges that there are
I would say your estimate of 10 exchanges is on the low end, but it's really a good thing, as the better ones will remain and the crappy ones will shut down

Maybe just because there is more than one person that wants to make some profit with fees.
Reading your signature, maybe you wann have a look at https://bitcointalksearch.org/topic/annexchange-alcurex-v201-458732
It will be a new exchange, the first with leverage for altcoins as far as i know.
You can buy fee / company shares.

Every time I am interested in a coin, it seems like it is on a different exchange!

There must be at least 10 alt exchanges.

I don't get it, because in most marketplaces there is diversity but one starts to outpace the others and dominate. I'm not sure which ones are dominating, or will dominate, in the altcoin market.
