Someone with wrong time on the PC.
It can be a problem for him/her if he/she is solomining, but for the network it's a non-issue.

Blocktime doesnt mean that that block will actually get mined or staked. If diff goes up and a big miner drops the rest of the miners will have to deal with the difficulty.
When it comes to pos blocks at OC those only happen if there is a tx fee in the mempool.

No relationship, only similar name. That was a reason coin was rebranded OC. It is no longer called Onecoin.
