There is written that "Permission is required
The form is available only in the owner's corporate domain. "
Also I joined to twitter bounty on August 24th. But why has not the corresponding share been calculated so far? What's wrong? Please explain. Is the reason I didn't fill the above form?

The permissions are fixed and as of now yes you need to fill out the forms.
The bounty manager was writing a software script that would be able to accurately record all of the data automatically, but it was taking a fair amount of time to develop so it was decided to have you all fill out this form to record your retweets.

We have been blown away by all of your efforts and the sheer amount of all of you who have joined the twitter bounty campaign. The number weren't expected and we needed to rapidly adjust to the volume.
