After some consideration, we've decided that using Google Forms for every aspect might not be the easiest option for our participants.
The following bounties have been edited to make it easier for you.

We are offering our funding campaign participants 15% of the funds raised, at a max of $600k. This is not an ICO and is more comparable to a crowdfunding campaign in which the participants will get monthly payouts to their ETH address once the exchange launches, for a maximum of 18 months.
