I'm just throwing out ideas here. What algo did you try to mine cause for some reason some GPUs don't like certain algo (I personally don't know why). Make sure in the sgminer-fixed.conf file to have only one algo config in and running the batch file again. Just a thought. (:

In the mean time I tried an old version of sgminer, 4.1.0. I think it's called sph-sgminer_x13mod. It didn't blue screen but it also didn't seem to work. so I tried other versions of sgminer using just a command bat file all the way up to version 5.1.2.
The thing is it isn't blue screening with any of them as long as I use a simple, custom bat file. But it also doesn't have any mining display, the CMD window is just black. But westhash shows I am connected but at 0 Mh. And I could swear I saw it actually hashing at one point, just VERY slow at like 5kh.
Edit: So Onward... I figured out the command to log to a file because I can't see what the miner is doing and this is what I get:
