I would NEVER fill out that form.

I don't think anyone wants to. But from the wording of the email filling it out is a must. So either receive nothing or allow the US govt to put their sticky fingers on a percentage of it, whatever that may be.

We're dealing with Lawyers acting within the Law. Who knows if the IRS will take a chunk of our little settlements.

On a positive note, Perhaps there are some Large Legitimate Claimants who drop out of the Settlement Pool as they don't want the IRS on their ass.

The Fewer the Claimants, the Bigger the Pool.

No...The Lawyers get 30% Max. They even reduced some fees to Maintain that.

My Claim form has been accepted too.

Non US resident.

I've sent all my screenshots of my stuck balances, copy of identication, Adressess and Tier 2 acceptance mails and all the mails between me and Crypsty servicedesk.

I had 7.6 btc over there.

Probably sent a lot less than others, but 2 or 3 screenshots of my account balance with the "accounts are frozen" notice at the top of the page for date verification.

Why are we expected to submit a tax form when a payout is not income but merely returning to creditors what was stolen from them?

On the W-8 BEN form, any idea what Part 1 #7 is supposed to be?  Reference number for what??

How can I get the Court to review my papers and evidence instead? Do I have to be in the US?

All I can think of is that everyone in the know (including Silver Law), knew people could get "out" of Cryptsy at a huge premium through some coins.

Where your screenshots from January 2016?

If not, maybe that's why you were denied?

I just received Cryptsy Settlement – Deficiency Notice.

I'm surprised as I submitted all available documentation.

Anyone with BTC claim been accepted?

How did you document the BTC account balance?

All I had to work on was my "Confirm Your Withdrawal" emails from Cryptsy, which provide withdrawal amount and address. I assumed that with this mail containing the unique confirmation URI they would have proof of withdrawal attempt, that ultimately failed, as we all know.

My Cryptsy BTC account balance I have no idea how to document.

Does the Notice you received allow you to further substantiate your claim or was it an outright rejection?

I'm allowed to contest the rejection, but what additional documentation can I provide? They have the BTC withdrawal amount and the receiving BTC address which they will see never received this amount. Thus I have lost that amount which I claim.

I received my "CRYPTSY CRYPTOCURRENCY SETTLEMENT DETERMINATION OF RECOGNIZED CLAIM" e-mail a few hours ago.

I previously submitted screenshots of my Cryptsy account, a spreadsheet of every trade I did on Cryptsy, e-mails, all the info and pics I used for my level 2 etc.

Now all I have to do is read up on filing the IRS Form  Huh

The annoying thing is that I hadn't used Cryptsy for a very long time - so long that I had to unlock it. I should have taken it as an omen, but convenience over opening a new account with some inferior UI made me push on.

Submitting a screenshot of account is of course great when you have access to the account, but for those who realised the danger after the site went down have no such opportunity.

A spreadsheet is most definitely a must, but I didn't have it in time to include Cryptsy.

The e-mails - do you mean all of them - like "Confirmation of Registration", "Ticket Received", "Successful Authentication", "Confirm you withdrawal" ? How do they prove wallets/balances?

What do you mean by "info and pics I used for my level 2 etc"? Do you mean two-factor authentication details?

Is there any way to find the origin address of a transaction at all? I have the last incoming transaction from Cryptsy, but as I never deposited BTC to Cryptsy I don't know a way to find my Cryptsy wallet address.

I credit fellow users here on bitcointalk for motivating me to take screenshots back in January 2016 and prior. It was all the talk at the time.

I had always kept a spreadsheet for fun and to total trades, that went through with multiple buys or sells.

The level 2 I referred to is not 2FA. It means I was a registered user (not anonymous).
