Seth, the form is for FIREMINE. I have filled it out now, but I suggest you take this matter very seriously from now on and create forms for all of your scam projects and pay each and everyone back their money as soon as you can. This is no light matter. At best you will up end in jail.

does this still apply given that CS is now delisting all of your projects and there has been a trade stop on Altswap for a while now? what is the procedure for Altswap share holders?

Since the scammer has been identified and we can prove he stole thousands of dollars we should be able to recover part of our investment... also, I think we should contact his family or his school to see what are their position on the issue.
