Inter is more consistent that Milan and I wouldn't take the risk to bet on Milan. Inter will win the match, and I believe that it would be a revenge on what Milan did to them in Coppa Italia finals. Inter should be more serious in this match and not allow Milan win them the second time, because it will be a shame to them.

AC Milan are one of most successful clubs in Serie A and in Champions League, they are a most successful Italian club. Even without Conceicao, this is a derby and the confrontation between these teams has always been very difficult and unpredictable. The fact that Milan managed to win the last matches between these teams does not mean that they will do the same in this upcoming match, if their standings are the same, then I think that for Inter this victory may be more important because they are fighting with Napoli for the Scudetto. Inter have +16 points over Milan and on paper everything looks obvious, but the derby status probably introduces too much randomness, I wouldn’t bet on Inter winning either, although the odds are attractive. BTTS seems to be looking safe, but I'm not sure about the total over, both teams are good (relatively) in defense.
