I'm trying to remember the name of an exchange that existed and was created here between 2012-2015.
It was written by like 16 or 17 year old user, that was pretty active on these forums. That user ended up having to shutdown the exchange due to legal issues or something.

As far as I know, this exchange wasn't hacked, but instead it shut down and refunded everyone.
I'm not completely sure but I think I found it:- Bitcoinica.com by "zhoutong"...
Note: I couldn't find the exact post that he/she mentioned about the age but a lot of people commented on the announcement thread that I linked earlier:
