Charles V was perhaps the greatest Emperor in European history, controlled like a third of Europe and most of the Americas.

Coming into ownership of a lot of lands by fortune of birth is not a sign of greatness in the sense being discussed... forging an empire out of nothing is. Those few individuals in history who were able to take a tiny nation and forge with it a vast empire are, in my mind, the only real candidates for "greatest emperor".
