Some cards do have high top up rates and also hit you with a couple of dollars surcharge to withdraw cash, most allow you to spend with no extra charges now though.

There are many card competing on the market now so fees will drop soon.

Could you share some debit cards that does not have a fee when you withdraw in a bank. I am really interested because I want to directly transfer my bitcoins in my debit card if that is possible.

Most credit/debit cards that are topped up by bitcoin should work at anywhere visa/masters are accepted. Else, why would we have them? It'd be pretty embarrassing to sell one and find out that they don't work.

If you are in a restaurant and you want to tip the waiter how would you do it, I have seen that waiters accept only cash so don't know if they accept credit cards tips. Another one is tip those that make shows like football tricks on the public streets in a city. The reason that we can't use in these places is that bitcoin is not designed to be used like cash (physically) so people should have maybe a mobile wallet so we can scan the QR code and send them a payment.

There are ways to get around the situations you mentioned.

Print off a couple of paper wallets and carry them with you, if you want to tip a waitress or street performer just pull out the piece of paper and use your phone to add the amount you wish to tip the person.
If they are not already into bitcoin then you have just educated someone new to the bitcoin world. If they are already using btc then they will most likely have a hot wallet on their phone and can offer you the QR code to scan.

Yes and I think confirmation time should not be issue with merchants as it may get better in coming years and merchants can deliver you goods once they see transaction in their wallet no need to wait till 3 confirmations.

Yes I agree that in order for mass adoption, Bitcoin should be spend as daily currency. It is like setting a role model on what to do with Bitcoin, since many people are not aware of it, it depends on the person who introduce on what will this people will do if they have bitcoin. If you show them as a currency then they use it as a currency. The problem that affect the usage of bitcoin is the absent of neighborhood merchant. No local store in the next street is accepting bitcoin for the majority of holder. Well I think to resolve this, one must start in putting up a local store in an area.
