I don't see there's a casino have all the criteria you mentioned above, but the closest one is Rollbit, they just don't have deposit bonus and free spins for new players. Rollbit has many PvP games, unlike other casinos who mostly only have slots, sports bookie and live casinos.

For 2,3, and, 4 that probably where it's get complicated especially regarding PvP considering this doesn't attract much in the long run even poker seems to be overlooked from time to time especially with the presence of bots nowadays. For 5 I think all casinos tend to have improved in this aspect but somehow depending on the network as well that you'll try to withdraw/deposit to when it comes to execution.
