This is not a phrase used in the EU so I have no idea what it means, But even if it's insulting I said no, As I dont know you, So would be wrong for me to say someting bad about someone I have had no dealings with.
I believe it's similar to the Baptist foot-washings of yore. Whenever someone acts to sanitize another, the sanitizer is in a subservient position to the sanitizee. So, I'm pretty sure "douchebag" means humble, helpful person. Now, the douchebagged may be in an entirely different position, where he has implicitly taken out a loan on good faith and credit of himself to eventually repay, though the douchebag will certainly insist it would be only a gratuity. Likely, the douchebagged will not repay the douchebag, making the bagged an asshole, and the bag a colonic.

This brings up an important debate among the philosophical intelligentsia, since many use "eat shit" as a pejorative command, when that is, in fact, opposite the reaction which should be brought by most systems of ethics. Always, we will have shit-eaters and those whose shit has been eaten, yet we decide those who have had their excrement consumed are superior to the excrement-consumers. Contrast that to general opinion on volunteer work, where Volunteers of America are shit-eaters, and those in poverty have had shit eaten from them with no means (likely no intent) to repay this debt. I would expect the general public to say something more like "Go have excrement eaten of you, then pass!" to be much more offensive than "eat shit and die."

I think what makes a shit-eatee something to be ashamed of is to not expect to make repayment; for one to have shit eaten of him, without intent to eat the shit of a shit-eater. I think, what industrialized nations of the world really need, are mandatory colonics included in their healthcare plans, which are to be administered by each citizen at random. The program I envision would be very similar to Jury Duty... Eating Duty.
