It is normal. Some masternodes who were created at the similar time can fight for signing transactions. It can cause few 15 blocks per day.
Not a big deal.

Correct or it can be just wallet with low conectivity. If wallet has low connection and finds a block...
It is not more than 1% of daily POS blocks.
