psy: awesome design. I really like it.

Why not fold it in thirds instead of in-half, that way the address is right there, but you have to unfold it to get to the private key. Psy's design would look awesome with the address on the "front" of the packet and the big B on the back. You can easily secure it with a piece of tape if you feel so inclined.

For those of us scrolling through the thread on our phones, it would be great to collect all the winners in the original post...just a thought...things like scrolling through 20+ pages or using the search feature is a major PITA.

Payouts!  Out of $200:

$130 goes to Psy for being the winner

$50 goes to Bit-Pay.  Factors leading me to put Bit-Pay in second place include responsiveness to adaptations.

$20 goes to the blue fractal design (but on HOLD pending conditions).  While very appealing and got voted more than Bit-Pay, I chose to put it in third place mainly because the appeal comes from an excellent selection of background art more so than iterating it to meet the needs which included an emphasis on vector.  This reward is also on HOLD pending someone showing me where this has been published in high resolution and the terms agreed to (and also I need confirmation that the background image that was used has a free license).  If it is not published and/or the terms have not been agreed to by now, and/or the background image is rights-incompatible with use in this project, then the $20 is split between the other two winners.

Please post payment addresses for the rewards.  Exchange rate used will be MtGoxLast at moment of transfer.

Mike is there a way we can have the user supply their GPG key and encrypt the private key so that in order to spent it he also needs to give the redemer the GPG thing to decrypt?

Really like #3 and #4, but I'll go with Psy's #4.

Practical consideration:  Should the "amount" line be visible when the private key is folded over?  It would be a shame to have a great looking note and then have to write the amount onto the folded back in white space.

For example, if the amount line was over the Load & Verify QR (where the equation currently is), it would be visible whether or not the private key was folded over.
