Its been long I get no more 20 free spins, today I checked my email and I see there is an email about another 20 FS promo as I used to get in the past.
But again, the old issue is coming back where I cant claim the FS code.
It means that the system is not working as it should be, I thought you have fixed it after I complained about it few times in the past.

Regarding the fees, we use an automated suggested fee system based on network congestion to deliver confirmations in a reasonable time to the user, this means sometimes we might pay just $1 in fees, but other times even $100+. The 1% fee we charge on PBG on average usually covers for that, but the past few months even the 1% on the bigger cashouts wasn't able to cover for all of our fee losses due to congested network.
Essentially, the 1% fixed fee on the side of the site, allows us to keep paying the smaller cashouts without absurd confirmation times needed, but even with those, we're generally at an overall loss.
We hope to be able to improve our fee structure in the future in such a way where the fees would be generally smaller (but confirmations could take days in a congested network), but users could pay a premium to ensure a faster confirmation time.

But also I can confirm that cashout transaction has total of 0.26 mBTC miner fee which is about $9-$10... I don't know where remaining $90 from cashout fee goes, probably to devs / owner / etc?

Regarding where the other $90 goes, I do not think you have the right to ask about it since it is part of their cashout rule where you are agree with it when you decide to play in this casino. They charges 1% of any cashout is indeed big amount and I believe this is the only site with such cashout fee system. Perhaps they need to consider to change it in the near future, at least make it competitive with other casinos. As I can see, the cashout fee system is one of the reasons why gamblers are not really interested to gamble here.

Don't know exactly but I think PBG meant that they aren't taking any commission from the withdrawal fee. And the 1% fee will be use to cover the network fee. However, in case of big withdrawal like this, 1% fee is more than needed. Look like they have processed the withdrawal of ‘TRXwhaleLOL’. Check the Top Cashouts at PBG, it shows a 306.45mBTC cash out of today. Which is the sum total of his withdrawal amount including the fee.

After logging into my account I have found that, the maximum withdrawal limit is 2000 mBTC. Seems like they haven't updated their FAQ page from a long period of time. His cash out amount is far away from the max withdrawal limit. Hopefully PBG will process his withdrawal quickly without making any issue. They should also update their FAQ page, otherwise it will create confusion like this.
