If this is a debit card, why is there a $60 fee? How do I fund it with bitcoin or western union? Whats the procedure?

you can use the card yourself to link it for example to coinbase or localbitcoins or any other exchanger to
receive money to the cards bank account when you sell BTC.
That is what I ment. I dont exchange - I just sell the cards.

on the card is your name embossed, so if you go to some place
like a restaurant or store to buy something - they might check the signature
on the back of the card.
If you need the card just for ATM or onlineshopping you will not need
a signature.
I use it myself but I did not sign it, so far no one asked.

the card and the PIN are both in sealed and original envelopes sent from the bank.
Only the customer will open the envelopes.

You say PIN in seal envelope. Does mean you know PIN ? If it mean that, what stop you from withdraw funds after user fill them up in few week or month ?
